Please follow these steps to apply for a Workers Permit:

1. Download form AT-17. Complete only Part 1 with a parent or guardian signature.
   
   
2. You will need a current physical dated within 12 months of the application date or the competed physicians form AT-16.
   
   
3. Email both documents to Debbie Affeldt or call the EMCC Counseling Center at 387-3816, to set up an appointment to receive your permit.
